چکیده

Abstract Background Alactic base excess (ABE) is a novel biomarker that estimates the renal capability of handling acid–base alterations during sepsis. Hence, aim this study was to evaluate use ABE predict replacement therapy (RRT) in patients with septic shock. Methods A total 164 admitted intensive care units diagnosis shock according third international consensus on sepsis and (Sepsis-3) were included. This retrospective, single center, conducted between January 1, 2016, December 31, 2020. The individuals stratified who did [ n = 68] or not 96] receive RRT. diagnostic performed variables for classification into required RRT evaluated by receiver operating characteristic (ROC) analysis area under curve (AUC) calculated. Univariate multivariate logistic regression models used identify risk factors Results median age 59 years female sex (51.8%) predominated. (odds ratio [OR] 1.2270, [95% confidence interval [CI] 1.0453–1.4403], p 0.0124) urea (OR 1.0114, CI 1.0053–1.0176], 0.0002) associated HCO 3 − 0.6967, 0.5771–0.8410], protective factor (AUC 0.649, < 0.0008), 0.729, 0.0001), 0.76, 0.0001) had cutoff point ≤ 5.7 mmol/L, 19.36 mmol/L > 75 mg/dL, respectively. Conclusion Although low risk, are independent

Partitioning evolutive standard base excess determinants in septic shock patients.

BACKGROUND AND OBJECTIVES The amount of metabolic acidosis measured through the standard base excess (SBE) has been shown to be an outcome marker and its improvement has been associated with better survival. We studied the mechanism of standard base excess variation in the first three days of intensive care unit (ICU) stay through the evaluation of independent variables of physico-chemical appr...

Renal replacement therapy neutralizes elevated MIF levels in septic shock

BACKGROUND Macrophage migration inhibitory factor (MIF) is known to amplify the immune response in septic animal models. Few clinical data support this pro-inflammatory role in septic patients. Renal replacement therapy (RRT) as adjuvants in the complex therapy of sepsis has been proposed as a possible approach to eliminate elevated circulating cytokines. Since recent data suggest that MIF can ...

Beta-lactam dosing in critically ill patients with septic shock and continuous renal replacement therapy

Although early and appropriate antibiotic therapy remains the most important intervention for successful treatment of septic shock, data guiding optimization of beta-lactam prescription in critically ill patients prescribed with continuous renal replacement therapy (CRRT) are still limited. Being small hydrophilic molecules, beta-lactams are likely to be cleared by CRRT to a significant extent....

Hydrocortisone therapy for patients with septic shock.

BACKGROUND Hydrocortisone is widely used in patients with septic shock even though a survival benefit has been reported only in patients who remained hypotensive after fluid and vasopressor resuscitation and whose plasma cortisol levels did not rise appropriately after the administration of corticotropin. METHODS In this multicenter, randomized, double-blind, placebo-controlled trial, we assi...

Albumin replacement in patients with severe sepsis or septic shock.

BACKGROUND Although previous studies have suggested the potential advantages of albumin administration in patients with severe sepsis, its efficacy has not been fully established. METHODS In this multicenter, open-label trial, we randomly assigned 1818 patients with severe sepsis, in 100 intensive care units (ICUs), to receive either 20% albumin and crystalloid solution or crystalloid solutio...
